Ask clients your questions before they book, and read their answers on each booking.

A Booking Form is a custom form clients fill in before they can book. Use it for intake questions, screening, health notes or consent.

When to use this

  • You need context before a session: goals, history, what they're struggling with.
  • You want to screen who books, or collect consent.
  • You'd rather not chase people for answers by email.

Set it up

Open the session type

Go to Offerings → 1:1 Sessions and open the session.

Add a form

In the Session details card, find Booking Form ("Collect additional information from customers before they book") and click Add Form.

Pick or create the form

Choose Use Existing Form and select one of your custom forms. Or click Or create new form, give it a Form Title, choose Contact Information (Required, Optional or Anonymous), and create it.

Decide on repeat bookings

Tick Allow one-time submission reuse if returning clients shouldn't fill the form in again. Their latest answers are reused instead.

Add your questions

If you created a new form, click Edit Questions to write them. You can also do this later from Audience → Forms & Funnels.

Once attached, the card says "Form attached" and "Form responses are shown in each booking detail page". Click Manage Form to swap or remove it.

Read the answers

Open any booking from the Bookings tab or View All Bookings. The form response appears on the booking page. In the bookings table, a form icon marks bookings that have a response.

Tips

  • Keep it short. Three to five questions is plenty. Every extra question costs you bookings.
  • Use Anonymous for sensitive topics. Responses are saved without a name or email.
  • One form per session. Attaching a different form replaces the current one.
  • Only custom forms can be attached. If your form isn't in the list, check that it's a custom form, not a lead form.
  • Want the form after booking instead? Use the Automation tab to email a form once someone books. See Send emails automatically after booking.

What your members see

After picking a time on the session page, they click Confirm & Pay (or Book Now for a free session). Your questions appear on the Confirm Booking page, above the payment. They answer them and click Confirm & Pay or Book Now to book. If they aren't signed in, they're asked to sign in first so they can answer.
