Collect info with a registration form
Ask intake, screening, dietary or intention questions that people answer before they can register.
You'll end up with a form attached to an event ticket. People fill it in before they register, and you see their answers on each registration.
When to use this
- Intake or health screening before a retreat or breathwork session.
- Dietary needs, room preferences or emergency contacts.
- Intentions or questions people want you to hold during the event.
- A waiver or consent people must give before they attend.
Steps
Open the Tickets tab
Go to Offerings → Events, open the event and click Tickets.
Add a form to a ticket
On the ticket, under Registration form, click Add Form.
Pick or create the form
Choose one of your custom forms from Select a custom form, or click Create Form to make a new one: give it a Form Title and choose whether contact details are Required or Optional. You can add more questions in Audience → Forms & Funnels.
Apply to other tickets (optional)
If the event has several tickets, use Apply to all tickets so everyone answers the same form.
Save
Click Save. The ticket now shows "Pre-registration form attached" and the button changes to Manage Form.
Read the answers
Open the Registrations tab and click a person. Their answers appear on the registration page with their ticket and status.
Tips
- Custom forms only. Lead-capture forms can't be attached. Make a custom form in Audience → Forms & Funnels first if you need one.
- One form per ticket. Attaching a different form replaces the old one.
- Keep it short. Every extra question is a reason to leave. Ask only what you need before the event; send a longer questionnaire afterwards with an automation.
- No file uploads in forms. If you need a photo or document, ask for a link (Google Drive, Dropbox).
- Form before vs. after. A registration form is filled in before registering. To send a form after someone registers, use Automate follow-ups.
- Remove it any time with Manage Form → Remove.
What your members see
The ticket button stays Register or Get Ticket. Your questions appear at checkout, above the payment. They answer them and click Register (or Register & Pay for a paid ticket). If they aren't signed in, they see "<your portal name> asks a few questions before this event. Sign in to answer them and get your ticket."
