Forms & Funnels
Run a feedback survey
Ask for ratings and honest feedback after a session, course or retreat, anonymously if you prefer, and read the results in one place.
A short survey after a session, course or retreat tells you what landed and what to change. A custom form with a few Rating and Paragraph questions is enough, and you can make it anonymous so people speak freely.
When to use this
- At the end of a course, program or retreat.
- After a first session, to see how it felt.
- To collect testimonials you can quote (with permission).
- To ask your audience what they'd like you to offer next.
Create a custom form
In Audience → Forms & Funnels, click Add Form, choose Custom Form and name it (for example "How was the retreat?"). For Contact Information:
- Anonymous for honest feedback: no names or emails are collected.
- Optional to let people choose.
- Required if you want to follow up or quote people by name.
Click Create Form.
Add a rating
Add a Rating question such as "How would you rate the retreat overall?" People pick one to five stars.
Ask open questions
Add two or three Paragraph questions: "What was most helpful?", "What would you change?"
Ask about next steps (optional)
A Multiple Choice question such as "What would you like next?" with your ideas as options helps you plan what to create.
Ask for a testimonial (optional)
Add a Paragraph question for a testimonial, followed by a Yes/No Question: "May I share this on my portal?" Only quote people who said Yes, and only if the form collected their name.
Share it
Click Share and send the link in an event blast, an announcement, or a message. To send it automatically when someone enrolls or registers, use an automation with Ask to Fill Out Form.
Read the results
On the Responses tab, the first two questions show as columns on a wide screen, so put the Rating first to scan scores quickly. Click See Answers for the full response, or Export Responses to work with the answers in a spreadsheet.
Tips
- Five questions or fewer. Long surveys don't get finished.
- Ask while it's fresh: the same day or the day after.
- Anonymous means anonymous. You won't be able to reply to someone's comment, so say so in the form's description if people might expect an answer.
- Soul has no reviews feature. Quote testimonials in your portal copy, a landing page or an announcement.
