Every question type a custom form can ask, how to add, edit, reorder and require questions, and what respondents see.

Questions belong to custom forms. A lead form always asks the same four things (First Name, Last Name, Email address and Phone Number, all required) and shows them on its Questions tab as "Lead capture fields" with a "Fixed structure" label. To ask anything else, create a custom form.

Add a question

Open the Questions tab

Open your custom form from Audience → Forms & Funnels. Under Form Questions ("Arrange questions in order and shape the submission flow here."), click Add question.

Pick a Question Type

Choose one from the list above.

Write it

Enter the Question Title (the question itself). Add a Description (optional) for help text, such as an example answer or why you're asking.

Add options (choice questions)

Single Choice, Multiple Choice and Dropdown need at least two Options. Type each one, click Add Option for more, and drag them into order. Empty options are dropped when you save.

Required or not

Tick Required if the form can't be submitted without an answer. Questions are optional unless you tick it.

Save

Click Add Question. It appears at the bottom of the list.

On a Yes/No Question, tick Require a "Yes" to continue: "Makes this a consent — answering "No" won't submit the form." The question gets a Consent label. If someone leaves it blank or answers No, they see "Select "Yes" to continue" and can't submit, whether or not Required is ticked.

Pair it with a Statement that holds the terms. See Collect a waiver or consent.

Edit, duplicate, delete and reorder

Each question card shows its type, a Required or Consent label, the title, the description and the first four options ("+[n] more" for the rest).

  • Edit: ⋯ → Edit opens Edit Question. You can change anything, including the Question Type.
  • Duplicate: ⋯ → Duplicate adds a copy, handy for a set of similar questions.
  • Delete: ⋯ → Delete removes it straight away, with no confirmation.
  • Reorder: drag a card by its handle. The contact block always stays first.

Before you reword or delete questions on a form that already has responses, click Export Responses so you keep a copy of the answers as they were given.

What respondents see

  1. Your form title and description at the top, with your portal name.
  2. A Contact Information block (unless the form is Anonymous): email and full name, marked (Optional) if you chose Optional.
  3. Your questions, in your order, each with its description. Required questions are checked before submitting, and anything missing is pointed out.
  4. A submit button. After submitting they see "Thank you!" and "Your response has been recorded," with an option to Submit Another Response.

Preview your form after every few changes. Reading it as a respondent is the fastest way to catch a confusing question or an option list that's too long.

Good to know

  • No conditional logic. Every respondent sees every question.
  • Questions can't be shared between forms. Each form has its own.
  • The first two answerable questions also show as columns on the Responses tab on a wide screen, so put your most useful questions near the top.
