# Collect a waiver or consent

> Build a form with your terms, a required "Yes" and a signature, and require it before an in-person event, a session or a retreat.

Breathwork, bodywork, plant-medicine circles, retreats and movement classes often need a signed waiver before someone takes part. A custom form can show your terms, insist on a "Yes" and collect a drawn signature, and you can make it a condition of booking or registering.

## When to use this

- An in-person event or retreat where you need a liability waiver.
- A 1:1 session that needs informed consent first.
- A program with rules or a code of conduct people must accept.

**Step 1: Create a custom form**

In [Audience → Forms & Funnels](https://portal.bringsoul.life/forms), click Add Form, choose
Custom Form, name it (for example _"Retreat waiver"_) and set Contact Information to Required so
every signature is tied to a name and email. Click Create Form.

**Step 2: Add your terms as a Statement**

Click Add question, choose Statement, and put your waiver's heading in Question Title and the
full text in Description. Statements show text with nothing to answer.

**Step 3: Add the consent**

Add a Yes/No Question such as _"I have read and agree to the terms above."_ Tick Require a "Yes"
to continue. The card gets a Consent label.

**Step 4: Add a signature**

Add a Signature question, for example _"Sign here"_, and tick Required.

**Step 5: Ask what you need to know (optional)**

Add questions such as health conditions (Paragraph), an emergency contact (Short Answer) or date
of birth (Date). Mark the important ones Required.

**Step 6: Test it**

Click Preview. Try answering No to the consent: you should see _"Select "Yes" to continue"_ and
be unable to submit. Then sign and submit, and check the response on the Responses tab.

**Step 7: Require it before the offering**

Attach the form where people book or register, and leave Allow one-time submission reuse off if
you want a fresh signature every time. See [Build an intake or application
form](https://help.bringsoul.life/luminaries/audience/build-an-intake-form#where-to-attach-it).

## What people see

Your terms, then the consent question with Yes and No (neither picked), then a box to draw their signature with a Clear button (_"Draw your signature in the box above."_). Choosing No, or leaving the consent blank, stops the form from submitting.

## Where to find signed waivers

Open the form's Responses tab and click See Answers on a response to see the consent answer and signature. Export Responses gives you a CSV of every response for your records.

## Tips

- Keep the waiver text in one Statement so it reads as a single document.
- Write the consent question in the first person: _"I agree…"_, _"I confirm…"_.
- Reuse is a choice. Allow one-time submission reuse is convenient for weekly classes; leave it off when each event carries different risks.
- Have your waiver wording checked by someone qualified for where you work. Soul collects the signature; it doesn't make the text legally sound.

**Warning:**
If the form is required and can't be submitted, nobody can book or register. Test the full flow in
a private window before you open bookings.
