A Custom Form asks exactly what you need to know: an intake questionnaire before a healing session, an application for a retreat, a waiver before an in-person event, a survey after a program. You can share it on its own, or require it before someone can book or buy. ## When to use this - You need health, background or consent information before working with someone. - You want people to apply before they can join a program or membership. - You want feedback after a session or course. **Step 1: Create a custom form** Go to [Audience → Forms & Funnels](https://portal.bringsoul.life/forms) and click Add Form. Add a Form Title and Description, choose Custom Form, and set Contact Information: Required (you always get their name and email), Optional, or Anonymous (for sensitive topics, nothing identifies them). Click Create Form. **Step 2: Add your questions** On the Questions tab, click Add question. Pick a Question Type, write the Question Title, add an optional Description (help text), and turn on Required if it must be answered. Repeat for each question and drag to reorder. **Step 3: Add consent or a signature (optional)** For a waiver, add a Statement with your terms, then a Yes/No Question with Require a "Yes" to continue turned on, or a Signature question. **Step 4: Send a follow-up email (optional)** Open the Automation tab to send the person an email after they submit, for example *"Thank you, here's what happens next."* **Step 5: Test it** Click Preview and submit it yourself. Check your answer appears under Responses. **Step 6: Require it before an offering** Attach the form where people book or buy. Turn on Allow one-time submission reuse if returning people shouldn't fill it in again. ## Where to attach it | Require the form before… | Where | | --------------------------------------------- | --------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- | | Booking a 1:1 session | The session type's settings. See [Onboarding](https://help.bringsoul.life/luminaries/business/onboarding). | Or simply share the form's link on its own with Share. ## The pre-purchase form window On a session type, an event ticket, a content item or a collection, the same window opens: Pre purchase form, _"Collect information from your audience before checkout."_ | Option | What it does | | ------------------------------- | ----------------------------------------------------------------------------------------------------------------------------------------------------------------- | | Use Existing Form | Pick one of your custom forms. Lead forms aren't listed. | | Or create new form | Create a custom form on the spot (Form Title, Description, Contact Information), then Edit Questions to add questions, or Do This Later. | | Allow one-time submission reuse | _"Reuse the latest submitted response for future purchases instead of requiring a new submission every time."_ Good for regulars; leave it off for fresh answers. | | Apply | Attaches the form. | Once a form is attached, the window shows Change Attached Form, the reuse option, Save, Edit Questions and Remove. Remove opens the offering up again with no form. **Note:** A form created from this window starts empty. Add at least one question before people reach it. ## Tips - Short forms get finished. Ask only what you'll actually use. - Ask for links, not files. Forms can't take uploads, so ask for a Google Drive, Dropbox or Instagram link. - One form per offering. Attaching another form to the same offering replaces the first. - Only custom forms can be required. Lead forms can't be attached before checkout. - Don't collect sensitive data you don't need. Use Anonymous for surveys on sensitive topics. **Warning:** If a required form can't be submitted, nobody can book or buy that offering. Always test the full flow in a private window before you share it. ## What people see When a form is required, people fill it in before checkout or booking. The offering is marked _"A form is required before purchase."_ If one-time reuse is on, people who already answered skip straight through.
